Job description
**Job Title: Registered Nurse**
Perform thorough nursing evaluations and work in partnership with the interdisciplinary team to create and execute personalized care plans for patients. Offer emotional support to both patients and their families, while also overseeing triage calls, patient admissions, and care transitions during after-hours shifts. A valid Registered Nurse license is required, along with previous experience in hospice, home health, or medical-surgical nursing. Applicants should possess excellent communication skills, be adept in electronic medical record (EMR) systems, and be available to participate in an on-call rotation, with access to reliable transportation.
