Job Summary
The Hospice RN serves as a pivotal clinical leader and case manager, dedicated to creating and executing individualized, comfort-centered care plans tailored to patients’ needs. This role involves collaborating with an interdisciplinary team to ensure comprehensive care while also offering emotional support and guidance to both patients and their families.
Responsibilities
- Develop and implement personalized care plans focused on patient comfort.
- Coordinate with an interdisciplinary team to ensure holistic patient care.
- Provide emotional support and guidance to patients and their families throughout the care process.
Qualifications
- Active Registered Nurse (RN) license.
- A minimum of one year of clinical nursing experience.
- Current CPR certification.
- A valid driver’s license and reliable transportation.
