**Job Title: Rehabilitation Specialist**
The Rehabilitation Specialist offers focused case management and rehabilitation services aimed at assisting clients and their families in navigating community resources and addressing challenges. As a key member of a multidisciplinary team, this role is responsible for executing personalized treatment plans and ensuring that services align with the specific needs of families. Applicants should possess at least 4 years of experience working in case management or rehabilitative services within a mental health context. A bachelor’s degree in social work, psychology, or a similar field is preferred, accompanied by excellent writing and communication abilities.
