The Field Case Manager visits members in their homes or long-term care facilities to assess health status and identify gaps in treatment plans. They provide patient education, coordinate care with interdisciplinary teams, and facilitate connections to essential support services. Candidates must hold a current, unrestricted RN license in Mississippi and possess at least one year of experience in community or public health nursing. A valid driver’s license and the ability to travel within the Greenville, MS area are required.
