Acts as a patient advocate to guide individuals through the clinical care system and facilitate timely treatment. Coordinates care interventions, manages patient intake, and collaborates with interdisciplinary teams to improve clinical outcomes. Requires a Registered Nurse (RN) license, with a Bachelor’s degree in Nursing and clinical experience preferred. Proficiency in electronic medical records and case management certifications are highly desirable.
