Job Summary
The RN Visiting Nurse Case Manager plays a crucial role in ensuring that nursing services are delivered effectively and safely to patients in their home environments. This position involves the creation, execution, and assessment of individualized care plans while collaborating with various healthcare professionals.
Responsibilities
- Ensure the safe and quality delivery of nursing care to patients at home.
- Develop, implement, and evaluate personalized care plans for each patient.
- Coordinate care efforts with other health professionals to address patient needs.
Qualifications
- Must be a Registered Nurse with at least one year of clinical experience.
- Previous experience in homecare is preferred, although not mandatory.
- Willingness to undergo on-the-job training to enhance skills in home care delivery.
