The Social Worker Case Manager assesses patient requirements and works alongside the interdisciplinary team to guarantee efficient care management and prepare for discharge. This role serves as a bridge between patients, families, and healthcare professionals to support smooth transitions and address any obstacles to care. Applicants are required to have a Master’s Degree in Social Work (MSW) and a minimum of one year of experience in clinical care or case management. A Basic Life Support certification must be obtained upon commencing the position.
