**Social Worker**
The role of the Social Worker involves offering counseling and assistance to patients and their families, aiding in effective transitions both during hospital stays and post-discharge. This position includes the creation and execution of detailed care plans, which encompass discharge planning and collaboration with external agencies. Applicants are required to have a Master’s Degree in Social Work along with a valid LCSW license. Candidates with one to two years of experience in either acute or long-term medical environments will be preferred.
