**Job Title: Social Worker**
The Social Worker is responsible for conducting thorough biopsychosocial evaluations, formulating diagnoses, and developing evidence-based care plans for patients in conjunction with an interdisciplinary team. This role involves advocating for patients to help minimize health disparities, while ensuring compliance with the NASW code of ethics and professional standards. Qualified applicants should have a Master’s degree in Social Work and either possess or be on track to obtain a Clinical Social Work license in Oregon. A minimum of two years of pertinent clinical experience in healthcare, hospice, or bereavement environments is essential.
