Physical Therapist
Clearfield, PA
13-week contract
M-F | 8:00 AM- 4:30 PM on-site
$2400/week
The position of Physical Therapist involves delivering patient-focused therapy services in Clearfield, PA, with a focus on outpatient rehabilitation and campus-based support. The therapist will primarily operate at the Outpatient Rehabilitation Center but may also provide assistance in Med Surg, Emergency Department, and Geriatric Behavioral Health as needed. Responsibilities include assessing patients, creating tailored care plans, administering specialized therapies, tracking progress, and collaborating with multidisciplinary teams to enhance functional recovery, ensure safety, and facilitate discharge planning.
Physical Therapist Responsibilities:
- Conduct evaluations of adult and elderly patients to analyze functional abilities, recognize mobility challenges, and set personalized short-term and long-term rehabilitation objectives.
- Design and execute treatment plans incorporating therapeutic exercises, gait and mobility training, strength development, range-of-motion activities, pain management strategies, education on body mechanics, and relevant modalities.
- Manage care for individuals with orthopedic issues, neurological conditions, cardiopulmonary problems, deconditioning, fall risk, post-surgical concerns, and geriatric behavioral health challenges, including conditions such as TKA, THA, TSA, CHF, Parkinson’s disease, dementia, and mood disorders.
- Maintain accurate documentation of assessments, follow-ups, discharge plans, treatment goals, and patient progress within Cerner, handling a daily caseload of around 10 patients.
- Work in partnership with Physical Therapy colleagues, Social Services, Rapid Response Teams, physicians, patients, and their families to ensure safe care delivery, enhance patient education, promote injury prevention, plan for discharge, and foster interdisciplinary collaboration.
Physical Therapist Qualifications:
- An active Physical Therapist license valid in Pennsylvania is mandatory.
- BLS certification is essential.
- Previous experience in outpatient clinics, acute care, orthopedic, geriatric, neurological, or rehabilitation environments is preferred.
- Familiarity with Cerner charting, experience in teaching hospitals, community hospital settings, and providing 1:1 patient care is advantageous.
- First-time travelers are encouraged to apply; Pennsylvania fingerprinting will be required prior to starting the position.
