Job description
**Job Title: RN Case Manager**
The RN Case Manager is responsible for performing thorough patient evaluations and creating personalized care strategies while facilitating access to medical and social support resources. This role includes delivering hands-on clinical care, such as home-based infusion therapies and remote patient monitoring, aimed at enhancing health outcomes and minimizing hospital admissions. Applicants must possess a Bachelor of Science in Nursing (BSN) and hold an active multistate Registered Nurse (RN) license. A minimum of two years of clinical experience in care management, infusion therapy, or community health is essential, along with a current driver’s license for travel purposes.
