Job Summary
The Nurse will engage in telemedicine consultations, execute clinical assessments, and oversee telehealth information to detect patterns of risk. This position requires collaboration with multidisciplinary teams to enhance chronic disease care and minimize unnecessary hospital admissions.
Responsibilities
- Conduct virtual patient consultations
- Perform clinical triage to assess patient needs
- Monitor telehealth data for identifying risk trends
- Collaborate with various teams to improve chronic disease management
- Work to prevent avoidable hospitalizations
Qualifications
- Active, unrestricted Florida LPN license
- Minimum of 2 years experience in home health care
- Competence in telehealth technologies and EMR systems
- Strong clinical judgment and decision-making skills
