Job Summary
The role involves providing comprehensive evaluation, diagnosis, treatment, and ongoing care to residents of skilled nursing facilities within the Concord region. This position also emphasizes the importance of patient education regarding their health conditions and care plans, ensuring that residents are well-informed about their treatment. The successful candidate will be responsible for maintaining accurate medical documentation, adhering to California healthcare regulations, and contributing to quality improvement initiatives and peer reviews.
Responsibilities
- Conduct evaluations, diagnoses, and treatment for healthcare residents.
- Provide follow-up care and educate patients about their health conditions and care plans.
- Ensure timely and accurate medical record maintenance.
- Comply with California health standards and organizational policies.
- Engage in quality improvement projects and participate in peer reviews.
Qualifications
- Graduation from an accredited Physician Assistant or Family Nurse Practitioner program.
- Current licensure in California.
- Valid DEA registration.
- CPR/AED certification.
- Demonstrated patient-care skills and proficiency with electronic health records (EHR).
- Strong communication and organizational abilities, professionalism, and empathy.
- Capability to collaborate effectively with a diverse range of patients and colleagues.
