The Case Manager serves to connect residents with the clinical teams and managed care associates, facilitating exceptional care delivery and operational efficiency. This role is crucial in managing various aspects of patient care and ensuring adherence to regulations.
Responsibilities
- Oversee and manage authorizations for patient services.
- Assist with the PDPM classification process.
- Coordinate safe and efficient discharge planning for residents.
- Ensure compliance with all relevant regulatory requirements.
Qualifications
- Possession of an active RN or LPN license in the State of Michigan.
- At least 2 years of experience in case management, utilization review, or MDS within a healthcare environment is desirable.
