Job Summary
The Home Health Field RN is responsible for delivering extensive nursing care, conducting assessments, and developing treatment plans for patients in their residences. This role involves working closely with interdisciplinary teams and providing education to both patients and their families to promote continuous care and achieve the best health results.
Responsibilities
- Provide comprehensive nursing services and assessments to patients in their homes.
- Develop and implement individualized treatment plans.
- Collaborate with multidisciplinary teams to enhance patient care.
- Educate patients and their families regarding health management and care continuity.
Qualifications
- Must possess a current Registered Nurse (RN) license.
- Hold a valid driver’s license.
- Certified in CPR.
- Preferred to have a minimum of two years of experience in home health care.
