Job Summary
The Nurse Practitioner or Physician Assistant (PRN) position focuses on conducting in-home health assessments in Saint Stephens Church, Virginia. The successful candidate will be part of Matrix Medical Network, a pioneer in delivering comprehensive in-home health care, encompassing evaluation, treatment, health education, and coordination for patients either at their homes or in skilled nursing facilities.
Responsibilities
- Perform in-home health assessments for patients.
- Deliver education and treatment plans to individuals and their families.
- Coordinate referrals for physician services and case management as necessary.
- Work collaboratively with healthcare teams to ensure comprehensive patient care.
- Provide follow-up care and support as outlined in treatment plans.
Qualifications
