Job Summary
The Patient Care Manager plays a pivotal role in supervising clinical operations and managing patient care initiatives, guaranteeing the delivery of excellent and compliant hospice services.
Responsibilities
- Oversee and coordinate clinical operations and patient care activities.
- Manage nursing staff and their performance.
- Conduct and facilitate interdisciplinary meetings to enhance patient care.
- Ensure compliance with federal and state regulations and standards.
Qualifications
- Must possess a valid RN license.
- A minimum of 3 years of direct patient care experience is required.
- At least 2 years of experience in hospice or home-based care is preferred.
- A bachelor’s degree in nursing is strongly preferred.
- Strong leadership capabilities and proficiency in clinical documentation are essential.
