Job Summary
The Social Worker will be responsible for conducting comprehensive psychosocial evaluations and crafting personalized care plans to assist patients and their families during their hospice experience. This role is essential in offering emotional support through crisis intervention and counseling, while also acting as an advocate for patients and working closely with a multidisciplinary team.
Responsibilities
- Conduct psychosocial assessments for patients and families.
- Develop and implement individualized care plans.
- Provide crisis intervention and counseling services.
- Act as an advocate for patients’ needs.
- Collaborate effectively with the interdisciplinary team.
Qualifications
- Master of Social Work (MSW) degree from a CSWE-accredited institution.
- Possession of an active LMSW or LLMSW license in the state of Michigan.
- Previous experience in home health or hospice care is highly desirable.
- Reliable transportation and a valid driver’s license.
