**Job Title: Skilled Home Health Nurse**
Delivers extensive nursing care and symptom management to individuals in their homes as an integral member of an interdisciplinary team. Works closely with healthcare providers to create and execute personalized care plans, ensuring precise maintenance of electronic medical records. Candidates must have at least two years of experience as a Registered Nurse in medical-surgical, home health, or hospice environments. A valid Registered Nurse license from Washington State, a driver’s license, and Basic Life Support certification are required.
