**Job Title: Care Coach**
Care Coaches offer tailored support, emotional assistance, and help in navigating resources for caregivers dealing with intricate health and social challenges. They are responsible for keeping precise documentation of coaching interactions and working closely with interdisciplinary teams to facilitate efficient care coordination. Applicants are required to possess a current professional license or certification in nursing, social work, or a relevant healthcare discipline. Additionally, a minimum of 5 years of experience in caregiver support or clinical care coordination is necessary.
