**Medication Case Manager**
The Medication Case Manager offers essential support and oversees medication management for individuals with psychiatric disabilities within an apartment treatment setting. Key responsibilities include evaluating client needs, connecting residents with necessary resources, and keeping precise records of the services provided. Applicants should possess an Associate’s degree along with 2 years of relevant experience or a Bachelor’s degree accompanied by 1 year of experience in case management focusing on mental health populations. A valid driver’s license, a clean driving history, and availability of a vehicle are mandatory for this role.
