Job Summary
The Care Manager is instrumental in supporting residents by facilitating their daily living activities, which encompass personal care, grooming, feeding, and assistance with mobility. This role is crucial in ensuring residents maintain their dignity and quality of life while receiving the care they need.
Responsibilities
- Assist residents with daily activities, including personal care and grooming.
- Provide feeding assistance and support mobility needs.
- Document services rendered and monitor any changes in residents’ conditions.
- Report significant changes to the supervising staff promptly.
Qualifications
- Must possess a Certified Nursing Assistant (CNA) or Certified Home Health Aid (CHHA) certification.
- Prior experience working with elderly individuals, especially those diagnosed with dementia, is highly preferred.
