Job description
The LPN Care Manager plays a crucial role in performing clinical assessments within patients’ homes and delivering continuous care management. This position involves working together with interdisciplinary teams to create tailored care plans and maintain compliance with regulations via scheduled home visits. Applicants must have a current LPN license in Missouri along with a minimum of one year of experience in geriatric or complex care. A valid driver’s license and dependable transportation are necessary for this field-based role.
