The Licensed Clinical Social Worker coordinates and implements post-discharge plans while facilitating referrals to community resources. They assess patient needs and collaborate with the interdisciplinary team to develop holistic, individualized care plans. Candidates must hold a Master’s degree in Social Work and possess a valid Licensed Clinical Social Worker (LCSW) credential. Proficiency in discharge planning, hospital operations, and knowledge of third-party payment systems is required.
