Job description
The Registered Nurse Case Manager is tasked with conducting patient evaluations, assessments, and implementing treatments based on personalized care plans. This role also entails maintaining precise documentation and promoting continuity of care within a hospice environment. Applicants must possess a valid, active, and unrestricted Registered Nurse (RN) license in the relevant state. The position requires the capability to carry out home visits and deliver specialized hospice care services.
