Job description
The RN Case Manager will create and modify patient care strategies while collaborating with an interdisciplinary team that includes LPNs and medical doctors. Responsibilities include performing home visits, carrying out OASIS assessments, and educating patients and their families. Applicants must have a valid RN license in Oklahoma and demonstrate strong assessment and critical thinking abilities. CPR certification is mandatory, and experience in home health care is highly desirable.
