**Job Title: RN Case Manager**
The RN Case Manager is responsible for delivering skilled nursing care to patients in their residences while supervising and coordinating home health services. This role involves working closely with the healthcare team to create care plans that focus on the clients’ goals and ensuring adherence to documentation and financial requirements. Applicants must hold a valid Registered Nurse license in the State of Minnesota and possess a minimum of two years of experience in healthcare or home care environments. A valid driver’s license, dependable transportation, and the capability to successfully complete background checks are essential for this position.
