Job description
**Hospice RN Case Manager**
The Hospice RN Case Manager is responsible for overseeing and coordinating patient care. This role involves performing assessments and creating customized care plans tailored to individual patient needs. The RN provides skilled nursing services, supervises hospice aides, and works closely with the interdisciplinary team to promote patient comfort and adhere to regulatory standards. Applicants should possess a valid Registered Nurse (RN) license and have a minimum of one year of experience in hospice or a similar healthcare environment. Essential qualifications include robust clinical assessment abilities, a valid driver’s license, and up-to-date CPR certification.
