Job Summary
The Hospice RN Case Manager PRN will be responsible for overseeing patient care in a hospice setting, ensuring that individuals receive compassionate and quality end-of-life care. This role requires frequent communication with the healthcare team and families to provide support and guidance throughout the hospice journey.
Responsibilities
- Conduct comprehensive assessments of patients’ needs and create tailored care plans.
- Coordinate with other healthcare professionals to ensure effective and cohesive care delivery.
- Educate patients and their families about hospice services and available resources.
- Monitor and evaluate patients’ conditions and adjust care plans as necessary.
- Provide emotional support and guidance to patients and their families during the hospice process.
Qualifications
- Current Registered Nurse (RN) license in the applicable state.
- Previous experience in hospice or palliative care preferred.
- Strong communication and interpersonal skills.
- Ability to work independently and manage multiple priorities in a dynamic environment.
- Compassionate and empathetic approach to patient care.
