**Job Title: Mental Health Professional**
The Mental Health Professional conducts assessments, diagnoses conditions, offers treatment, and provides crisis intervention services for clients, ensuring effective coordination of care to promote community well-being. This role involves overseeing utilization practices, planning for patient discharge, and serving as a bridge among patients, referral entities, and funding sources. A Master’s degree in Social Work, Counseling, or a similar discipline is essential, alongside a valid Pennsylvania license as an LCSW, LPC, or LSW. Applicants should have a robust understanding of community resources and be capable of performing physical tasks associated with patient care.
