Job description
**Job Title: Nurse Navigator**
The Nurse Navigator plays a pivotal role in offering thorough coordination of care for individuals undergoing structural heart interventions. This role includes patient education, clinical evaluations, and developing treatment strategies. Additionally, the Nurse Navigator oversees the transitions of care, ensures adherence to registry protocols, and works closely with various teams to promote patient safety. Applicants should possess a Bachelor of Science in Nursing and a valid California Registered Nurse license, in addition to CPR and ACLS certifications. A minimum of two years of experience in cardiovascular or hospital nursing is required, with a preference for candidates who have backgrounds in critical care or case management.
