Job Summary
The primary role of this position involves performing thorough primary care visits and physical examinations within patients’ homes, specifically targeting the management of chronic illnesses. This role emphasizes a collaborative approach, working alongside an interdisciplinary team to facilitate smooth transitions of care and to minimize avoidable hospital admissions.
Responsibilities
- Conduct comprehensive primary care assessments and physical evaluations in the home setting.
- Manage and monitor patients with chronic diseases effectively.
- Work collaboratively with a diverse healthcare team to enhance patient care continuity.
- Implement strategies aimed at reducing the risk of unnecessary hospitalizations.
Qualifications
- A Master’s degree in Nursing, specializing as a Nurse Practitioner with a focus on Adult, Geriatric, Acute Care, or Family practice.
- An active, unrestricted Nurse Practitioner license.
- Board certification as a Nurse Practitioner.
- A valid driver’s license for necessary travel to patients’ homes.
