The Nurse Practitioner or Physician Assistant will deliver essential clinical services, encompassing urgent care and infectious disease assessments, during mobile outreach operations. This role involves working closely with social workers to ensure that homeless patients are linked to vital medical and social resources. Applicants should have a valid Illinois APN or PA license and a minimum of 5 years of relevant clinical experience. Certification in BLS/ACLS and possession of a valid driver’s license with dependable transportation are also mandatory.
