Job Summary
The role focuses on performing thorough in-home health evaluations for adult and elderly patients, assessing aspects such as medical histories, adherence to medication, and potential social risks. This position involves cooperation with primary care physicians and caregivers with the aim of enhancing health outcomes and addressing care deficiencies.
Responsibilities
- Conduct comprehensive in-home health assessments for adult and geriatric patients.
- Evaluate patients’ medical history, medication compliance, and social risk factors.
- Collaborate with primary care practitioners and caregivers to foster improved health results.
- Identify and address gaps in care to support patient health management.
Qualifications
- Master’s degree in a relevant field.
- Current Nurse Practitioner licensure with the authority to prescribe medications.
- Board certification in a pertinent medical specialty.
- Active certifications in BLS, ACLS, or CPR.
