Job description
The Nurse Practitioner will deliver thorough follow-up care for patients after their discharge from the hospital through phone and video consultations. Key responsibilities encompass medication reconciliation, assessing symptoms, and managing care across multiple healthcare environments. Applicants should hold a certification as a Nurse Practitioner in Adult, Family, or Geriatrics, along with a valid Pennsylvania license. A minimum of five years of clinical experience following graduation in either Primary Care or Geriatrics is mandatory.
