**Job Title: Nurse Navigator**
The Nurse Navigator is responsible for delivering exceptional education, ongoing follow-up, and emotional support to patients and their families in both inpatient and outpatient environments. This role also includes overseeing clinical programs and engaging in quality improvement initiatives to evaluate outcomes and ensure seamless continuity of care. Applicants are required to possess a Bachelor’s degree in Nursing and a minimum of 5 years of prior nursing or clinical experience. Essential certifications include a Michigan RN license and Basic Life Support (BLS); additional certifications in ACLS or PALS may also be necessary.
