The Physical Therapist will provide individualized patient care and educate families in a home environment. Their primary responsibilities include evaluating patient requirements, creating tailored treatment strategies, and executing therapeutic interventions aimed at enhancing functional capabilities. Applicants should have graduated from a recognized physical therapy program and possess an active state license. Experience in Home Health and familiarity with Oasis documentation is advantageous, as is having a current CPR certification and dependable transportation.
