Job description
Conduct in-home physical therapy sessions that involve evaluations, reevaluations, and assessments related to fall risks. Ensure precise documentation of patient progress and maintain clear communication with the healthcare team. A valid Hawaii license as a Physical Therapist or Physical Therapist Assistant is mandatory. Applicants should have a minimum of one year of clinical experience and must hold a valid driver’s license along with dependable transportation.
