**Job Title: In-Home Physical Therapist**
Deliver in-home physical therapy services, which encompass initial evaluations, reassessments, and assessments of fall risks. Ensure precise documentation and facilitate effective communication regarding patient progress with the healthcare team. Applicants must possess a valid Hawaii license as a Physical Therapist or Physical Therapist Assistant. A minimum of one year of clinical experience, ideally in a home health setting, is necessary, along with a current driver’s license.
