Job Title: Primary Care Provider for Seniors
Focus on delivering preventive healthcare services to adults aged 65 and above, emphasizing the management of chronic conditions. Collaborate within a multidisciplinary care team that includes Nurse Practitioners, Physician Assistants, social workers, and support personnel to provide comprehensive, value-driven healthcare. Candidates must hold Board Certification or be Board Eligible in either Internal Medicine or Family Medicine. A willingness to engage in a value-based care environment and excellent communication abilities are essential for this role.
