Job description
As a provider, deliver preventive primary care services for individuals aged 65 and above within a value-focused healthcare framework. Work alongside a comprehensive care team that consists of Nurse Practitioners, Physician Assistants, social workers, and pharmacy professionals to effectively oversee the management of chronic conditions. Candidates must hold Board Certification or be Board Eligible in either Internal Medicine or Family Medicine. Experience in geriatrics and a background in value-based care is advantageous, though not mandatory.
