Job description
**Job Title:** Primary Care Physician for Geriatric Patients
Deliver proactive primary healthcare and manage chronic conditions for individuals 65 and older within a value-driven care framework. Work alongside a multidisciplinary care team, which includes Nurse Practitioners, Physician Assistants, social workers, and other support personnel, to enhance patient wellness and coordinate care effectively. Candidates must hold Board Certification or Board Eligibility in Internal Medicine or Family Medicine. While experience in geriatrics, value-based care, and HEDIS measures is desirable, it is not mandatory.
