**Start of Care Nurse**
The Start of Care Nurse delivers exceptional, patient-focused nursing care through initial assessments and the management of tailored care plans in home settings. Responsibilities include administering treatments, tracking patient progress, and working closely with the healthcare team to provide holistic care. Applicants should possess a valid Registered Nurse (RN) license and demonstrate robust clinical expertise, with a preference for candidates experienced in home health or wound care. The position demands physical capability to lift up to 50 lbs, alongside strong interpersonal skills for effective communication with patients and their families.
