Job description
Deliver comprehensive primary care that encompasses preventive measures, chronic illness management, and health education for patients. Function autonomously using a relationship-centered approach to medicine while leveraging the resources provided by the organization to achieve optimal patient outcomes. An active license and board certification as either a Nurse Practitioner or Physician Assistant are essential. Applicants should possess experience in independent family medicine, including regular wellness visits and urgent care services.
