**Job Title: RN Home Health Case Manager**
The RN Home Health Case Manager carries out detailed nursing evaluations and formulates personalized care strategies in collaboration with an interdisciplinary team. This role involves delivering direct skilled nursing care, overseeing support personnel, and guaranteeing compliance with documentation standards while tracking patient outcomes. Applicants must have completed an accredited nursing program and possess an active registered nurse license. A minimum of one year of nursing experience, current CPR certification, and a valid driver’s license with insured transportation are essential requirements.
