The Registered Nurse will oversee patient care throughout their entire journey, from admission to discharge. This role includes conducting OASIS assessments and creating personalized care plans tailored to each patient. The RN will work alongside a multidisciplinary team to deliver skilled nursing services and maintain adherence to Medicare standards and agency protocols. Applicants should have an active Ohio RN license and a valid driver’s license, along with dependable transportation. Although prior experience in home health and familiarity with OASIS assessments is advantageous, it is not mandatory.
