Qualifications

- Documentation of Influenza Vaccine

- 2 Step PPD Test OR IGRA (Quantiferon/Tspot) results
- Submission of UHS Submission Packet
- UHS Badge Photo required
- Criminal Background Check Results through UHS
- UHS OIG Search verification
- Screening against SAM/EPLS/GSA/OFAC lists
- Passing UHS Drug Screening
- Completion Certificate for UHS Module
- Prior Employment Form from UHS
