Job Summary
The LPN will play a crucial role in managing chronic care referrals and providing prescribed medications and treatments under the oversight of a physician. This position is key in ensuring effective communication among the healthcare team and with patients, alongside maintaining precise patient medical records.
Responsibilities
- Coordinate referrals for patients with chronic care conditions.
- Administer medications and treatments as prescribed, under physician supervision.
- Maintain accurate and up-to-date patient medical records.
- Facilitate effective communication between patients and the healthcare team.
Qualifications
- Vocational/Technical Diploma in Practical Nursing.
- Valid LPN license in Virginia.
- At least 2 years of nursing experience in a hospital or clinic environment.
- Current BLS (Basic Life Support) certification.
