Job Summary
The Registered Nurse for Home Health Visits is responsible for providing a range of nursing care to patients in their homes. This critical role aims to ensure that patients receive personalized and high-quality care in a comfortable setting while assisting them in achieving optimal health outcomes.
Responsibilities
- Conduct comprehensive patient assessments and evaluations in a home environment.
- Administer medications, wound care, and other necessary treatments as prescribed.
- Educate patients and their families about care plans and health management.
- Collaborate with a multidisciplinary team to coordinate care and ensure continuity.
- Document patient progress and modifications to treatment plans accurately.
- Provide emotional support to patients and families during care delivery.
Qualifications
- Current Registered Nurse (RN) license in the state of practice.
- Experience in home health care or a similar nursing field preferred.
- Strong assessment and clinical skills.
- Ability to communicate effectively with patients and their families.
- Experience with electronic health records (EHR) is a plus.
