Job Summary
The Health and Porter Medical Center is currently on the lookout for a committed and skilled Registered Nurse (RN) or Licensed Practical Nurse (LPN) to join our Memory Care team in Middlebury, Vermont. This role is crucial for delivering exceptional patient care in a rehabilitation and nursing environment. The selected candidate will be responsible for following physician directives and providing high-quality nursing services to our patients.
Responsibilities
- Provide nursing care to patients in accordance with established protocols.
- Implement physician orders accurately and timely.
- Monitor patient health and report any changes to the healthcare team.
- Assist with the development and implementation of patient care plans.
- Educate patients and their families about health conditions and care plans.
Qualifications
- Current licensure as a Registered Nurse or Licensed Practical Nurse in Vermont.
- Experience in memory care, geriatrics, or related fields preferred.
- Strong communication and interpersonal skills.
- Commitment to providing compassionate and respectful care to patients.
- Availability to work onsite in Middlebury, Vermont.
