We are seeking a Nurse Practitioner or Physician Assistant to join our team on a PRN basis for in-home health assessments in Strong, Maine. This role involves conducting evaluations and providing medical advice directly within patients’ homes or in Skilled Nursing Facilities. The ideal candidate will be committed to delivering exceptional healthcare and enhancing patient outcomes.
Responsibilities
- Conduct in-home health assessments to evaluate patient health status.
- Provide health education to patients and their families.
- Facilitate physician referrals as needed for further care.
- Collaborate with case management to ensure coordinated patient care.
Qualifications
- Current certification as a Nurse Practitioner or Physician Assistant.
- Experience in home healthcare or similar settings is preferred.
- Strong communication and interpersonal skills to engage with patients.
- Ability to work independently and manage time effectively.
